Remove ID metadata fields from definition files 
They are not being used any more. For some reason people thought that an exception was made for extruders or something. And people keep adding them for the definition files too. I'll add a test to prevent that.

afb83fac88 Update the active extruder index when the global container changes. This 
fixes a problem when Cura starts with a single extruder printer but the
user then choose a multiextruder printer - CURA-4386
Also added a quality definition in UM3E extruder with the ids of the UM3
extruders, so that the profiles for UM3 are also valid for UM3E and viceversa - CURA-4389
All are related issues with profiles and multiextruder printers

Change dimensions of Ultimaker 3 and reset nozzle origins 
The left nozzle is at the origin again now. And the right nozzle is at an offset of 18mm of that.

Remove superfluous disabling of prime location settings 
These are already disabled in the definitions of fdmprinter and fdmextruder. It doesn't need to be disabled again.

Publish Ultimaker 3 definitions and profiles 
The Ultimaker 3 is now released, so we can make this data public. These are the Ultimaker 3 definition files (with their physical properties as Cura requires them, such as build volume) as well as its extruders, print cores, quality profiles and build plate mesh. And also for the Ultimaker 3 Extended. Modifications to the material profiles will be published in the fdm_materials repository.
