Merge machine_head_polygon and machine_head_with_fans_polygon defs.
...
There is no usecase for having both. when a definition has both,
the head is removed. when it has only head, it is renamed to
_with_fans

Overwrite gantry height value instead of default_value

Because the default in fdmprinter has a formula in the value, the default_value properties wouldn't get used any more.

Only set top/bottom pattern to concentric for initial layer
...
I'm taking responsibility for this. I'm changing it as a reaction to #4167 since I think concentric is good for the first layer but not so good for the rest of the layers. That user seems to agree but it hasn't been tested thoroughly. It's my intuition, sorry if it went wrong.

Fix default quality level for Creality CR-10 printers
...
Instead of having a default layer height of 0.2mm, we change the default quality profile to be the one with a layer height of 0.2mm. This fixes up the quality slider.
